Ebenezer Deer Crest will begin a partnership with Bluestone Physician Services next month.

Bluestone provides on-site primary care to residents in assisted living and memory care communities.

The partnership will include frequent on-site visits and ongoing communication through an online portal. This allows for patient health to be monitored carefully and for care plans to be customized, the company said.

Mickie Schutz, a community life coordinator at Potter Ridge, recently was named the Aging Services of Minnesota 2014 District F Caregiver of the Year.

Schutz’s co-workers nominated her for the award, citing achievements such as an exercise class, developing the Potter Ridge Singers and getting residents involved in community projects.

“The Caregiver of the Year award is about honoring the very best of the best,” said Gayle Kvenvold, president and CEO of Aging Services of Minnesota. “This award provides us with the opportunity to show Minnesotans the face of the people who care for our loved ones when we cannot. Mickie Schutz is the kind of caregiver we all hope will be there for us when we need help.”
